What is MyMemorial Patient Portal?
MyMemorial Patient Portal is a free, password-protected and web-based interactive service that allows you to securely access and track your health information. Using your personal computer or smart phone, you can sign in to MyMemorial and see your test results, medication you are taking and a summary of your current health conditions.
What kind of health care information will be available to me through MyMemorial?
MyMemorial allows you to view a summary of your health information based on your visits to Memorial. You can view your test results, see your appointments (both future and in the recent past), treatments you have received such as immunizations, any allergies you may have, and what medications your records show that you are taking.
How do I sign up?
Register for the portal by visiting the Health Information Management (HIM) Department. A valid email address and photo ID are required for registration. Within a few days, you will be sent an email with a link to the MyMemorial web page. This link will include a temporary log on and password. Once you click on the link and follow the prompts, you will be an active user in MyMemorial.
Is my information safe?
Yes. MyMemorial passwords are encrypted so that they cannot be copied and pasted. You and authorized family members are the only ones who can access your Portal. Also, a timeout features protects your information if you leave the portal page open. User name and passwords are required.
What should I do if I forget my username and/or password or have technical problems with the portal?
Please contact us at 618-257-5300 to re-establish your user name credentials. We are available Monday through Friday during normal business hours. If you are logged into the portal, you can also submit your questions via the “Contact Us” feature on the various pages of the MyMemorial Portal.
To reset your password, this can be done on the log on screen by clicking on “Forgot Password?” You will be asked for your log on ID and email address. A new one-time password will be emailed to you.
Can I access the Portal via a mobile app?
Is there an app that you recommend that would work with MyPatient Portal?
No. Memorial does not endorse any third party apps, however, you may submit a request to Memorial Health Information Management for specific app approval.
Where does my health information in the Portal come from?
All the information is in the MyMemorial Patient Portal comes from your Memorial Electronic Health Record. This ensures that you have access to the most accurate, up-to-date information possible.
What medical documents can I access through MyMemorial?
Results from most laboratory and radiology tests performed at a Memorial facility are available through MyMemorial. Please note that radiology reports, not images are available. In addition discharge instructions and eHealth Visit summary documents are available to patient who recently received care at Memorial.
How soon will I see my documents?
Most documents will be posted to MyMemorial within 72 hours of completion. This short delay allows time for your provider to review your results and share them with you. Please keep in mind that some tests take longer to complete than others and these results may take longer to be posted.
What if I have a question about my results? Who do I contact?
For questions about specific test results, please contact the health care provider who ordered the test.
How do I access other family member’s records?
Connecting your MyMemorial account to another account such as a family member is called “proxy access”. Your family members must grant you access to their MyMemorial accounts, if they choose. If you would like to become a proxy to another patient’s account, or assign a proxy to help manage your own account, please visit with the family member our HIM (Health Information Management) department with a photo ID.
How do I access my child’s records?
If your child is under the age of 12 a parent/guardian can have access to their child’s records. Completion of the proxy access form will be necessary. Parent/guardian access automatically expires on the child’s 12th birthday. This is required by Illinois state law. Parents and legal guardians may still request paper copies of all medical records for their children under 18 from Memorial’s Health Information Management.
